Dr. Lilian Alevato Medical Director of Clinical Services

Dr. Lilian Alevato brings over 40 years’ experience as a healthcare provider in the fields of Internal Medicine, Cardiology & Geriatrics with additional specializations in Compliance, Quality, Utilization Management, Case Management, and Population Health.

Just after completing her Residency studies, she became a Hospital Director of a Community Hospital leading a group of over 80 physicians in Rio de Janeiro for 12 years until she decided to continue her studies in Healthcare Administration in the U.S.A. to continue her career in Managed Care Administration.

Dr. Alevato completed her master’s in business healthcare administration at COPPEAD – Federal University of Rio de Janeiro with extension program at John Hopkins School of Medicine. She also completed her master’s in public health at the Brazilian Army in the last year practicing in Brazil.

In the year 2000, Dr Alevato decided to move to United States to continue her studies in Quality, Compliance and Utilization Management.

Dr. Alevato holds several certifications with the American Institute of Healthcare Compliance (AIHC); National Committee of Quality Assurance (NCQA) PCMH-CCE; she is Board Certified Physician Advisor by the American Board of Quality and Utilization Review Physicians in the subspecialties of Transitions of care, Managed Care and Physician Advisor.

Dr Alevato was also selected as a Peer Leader Speaker for the CMMI Centers of Medicare and Medicaid Innovation on the BPCI Advance program in the year 2021, for the best Care Plan Design selected by CMS.

Dr Alevato’s passion for value-based care continues to drive her efforts in developing new programs to educate physicians, case managers and health care professionals to work as a team to improve patient’s quality of life and better utilization outcomes.

Wade A. Brosius Chief Medical Officer

Wade A. Brosius, D.O. is the Chief Medical Officer for PMSI, a vertically integrated multi-specialty group in suburban Philadelphia.  He has held this position for the past year.  He is also a family physician at Springford Family Practice, a division of PMSI, located in Royersford, PA.  He has been in practice for 29 years at this location.  Dr. Brosius is also Medical Director for Accurate Coding and Education for Tandigm Health in Conshohocken, PA.  Dr. Brosius also serves as Medical Director of Suncrest Hospice, Philadelphia.

Dr. Brosius earned his Bachelor of Arts degree in Biology (1988) at La Salle University.  Dr Brosius received his medical degree at Philadelphia College of Osteopathic Medicine (1992).  He completed his internship and family medicine residency at the Hospital of the Philadelphia College of Osteopathic Medicine where he served as Chief Resident (1995). 

Dr. Brosius has expertise in risk adjustment coding and documentation as well as population health management.  He has lectured extensively on these topics locally and regionally. 

Dr. Brosius is very active in the medical community.  He sits on the Executive Committee of PMSI.  He also serves as the Chairperson of the Quality Committee as well as Diabetes Committee and sits on the Medical Records Committee for PMSI.  Dr. Brosius is the past Chairperson for the Department of Family Medicine at Pottstown Memorial Medical Center where he also served on the Medical Executive Committee for many years.  

Dr. Brosius also is active in the community where he lives.  He is the team and school physician for Pottsgrove School District.  He is also the school physician for Springford School District and Pope John Paul II High School.  He attends mass regularly at Sacred Heart parish in Royersford, PA, where he serves on numerous committees.

Dr. Brosius is married to Gretchen Ann Brosius for thirty two years.  They are the proud parents of two sons,  Michael (27) and Jack (22).

Dr. Shannon Decker is principal at VBC One, a consulting firm assisting physician groups, health plans and vendors maximize the performance of their value based contracts and offerings.  Dr. Decker has more than 20 years experience in health care and most recently led teams in risk, quality, data and analytics, telehealth, COVID response and delegation compliance.  Of her more than 20 years of experience in healthcare--15 include working with risk adjustment, quality and Medicare. Dr. Decker has a PhD. in Interdisciplinary Studies, dual MBA degrees--in Finance and in Marketing, as well as an M.Ed. in Secondary Education and a M.Ed. in Administration and Leadership. Dr. Decker is on the faculty at Arizona State University and is also an associate professor of Higher Education & Adult Learning (HEAL) and chief methodologist for Walden and Capella Universities where she chairs and oversees the dissertations of doctoral students. An author of two books and several peer-reviewed articles, and a prolific national public speaker, her interests include the study of human behavior and how theories on motivation and learning may be brought to bear on population health management.

Lance Donkerbrook Chief Executive Officer

Mr. Donkerbrook has over twenty five years of healthcare experience ranging from consulting, business planning, financial management and contracting within national health insurers, hospital delivery networks, physician practices and healthcare technology companies. Outside of his more recent ACO leadership, Lance has implemented many innovative projects related to strategic planning, system selections, business case, process reengineering, productivity, and contracting associated with health plan operations and their IT systems implementations.

For the last five years, Lance has led as the COO, and more recently, elected as the CEO of the Commonwealth Primary Care Accountable Care Organization based in Tempe, Arizona. He has a BS in Economics from Arizona State University and an MBA from Texas Christian University. Lance serves as an advisor for three technology companies, and volunteers on the the Men’s Arts Council with the Phoenix Art Museum.

Carolina Espinoza-Strelec, MPH, CMQ, CPHQ, currently serves as the Program Director for the Ascension Seton Health Alliance (SHA) and the Ascension Medical Group, Texas (AMGTX). In the SHA, Carolina is responsible for population health, clinical integration, and payor contracting. SHA is the largest clinically integrated network in Central Texas with over 4,000 providers, managing over 230,000 value-based lives through two Medicare Shared Savings Programs, multiple commercial models and a direct-to-employer model. In AMGTX, Carolina oversees quality, patient safety, and performance improvement for approximately 190 multidisciplinary clinics across Central Texas. Before joining Ascension Seton, Carolina spent 10 years in acute care infection prevention as System Director for the Memorial Hermann Healthcare System in Houston, Texas. 

Carolina is passionate about implementing clinical quality, prevention, and population health management strategies that improve patient care, outcomes, and experience while finding ways to reduce the overall cost of healthcare. Carolina is also passionate about providing clinicians and staff with resources and tools that assist in improving efficiency to allow for more meaningful patient-clinician relationships. 

Carolina holds a Bachelor of Arts in Biology from the University of Texas at Austin and a Master of Public Health Epidemiology from the University of Texas Health Science Center in Houston.

Ava Johnson is the Associate Director of the Outpatient Clinical Documentation Quality Improvement department at Mount Sinai Hospital in NYC. Ava is a seasoned coding instructor, physician educator, multispecialty coder, CDI specialist and public speaker with a focus on executive leadership and career development.

Tanya Kapka Senior Medical Director, NYC

Tanya Kapka, MD, MPH is a leader in health care transformation for evidence-based, patient-centered primary care in service of the Quadruple (Quintuple) Aim. Her clinical and research experience and collaborative approach to implementation of high-impact, data-informed projects via multidisciplinary teams are where she is most effective. Her professional life for over two decades has prioritized leading and teaching innovative, patient-centered care delivery for publicly-insured patients and families facing social and structural barriers.

In Oregon for most of her career, she held Medical Director leadership positions at Virginia Garcia Memorial Health Center and at CareOregon, with a focus on innovative care delivery in value-based payment models for Medicaid and Medicare D-SNP. Since returning to her home state of New York a few years ago, she served as D-SNP Medical Director at Cityblock Health and then, until recently, as Population Health Medical Director at Mount Sinai Health System.

Still a practicing board-certified physician, Dr Kapka is now Senior Medical Director in NYC at Oak Street Health. A Fellow of the American Academy of Family Physicians, she also serves on the Public Health Commission of the NY State Academy of Family Physicians. Her MD is from Oregon Health and Science University and MPH from Harvard School of Public Health.

Since joining the organization in 2022, Inna has led the Medicare ACO (MSSP and ACO REACH), Commercial, and Medicaid risk products for the organization. Inna started her career at Evolent Health driving provider led health plan development and operations with health systems across markets. Most recently, Inna spent 5 years at Optum Health, part of broader Unitedhealth Group organization where she led new valued based market development with newly acquired provider organizations. A board certified neurologist, Dr. Paroski has held various administrative positions including Medical Director at Erie County Medical Center; Senior Associate Dean of Academic Affairs & Admissions and Interim Vice President of Health Affairs and Interim Dean at the Jacobs School of Medicine and Biomedical Sciences, University at Buffalo; and Executive Vice President and Chief Medical Officer at Kaleida Health.

She began her medical career at the Buffalo VA Medical Center and held hospital appointments at the Buffalo Psychiatric Center, Roswell Park Cancer Institute, Kaleida Health, and Erie County Medical Center. Kim Prendergrast Vice President of Policy

Kim Prendergast is Vice President for Policy for Community Care Cooperative (C3). a Federally Qualified Health Center-led Accountable Care Organization (ACO). In this role, she leads the ACO’s strategic and policy initiatives to support FQHCs across six states and the District of Columbia to leverage value-based payments and address social drivers of health to improve health equity and health outcomes.  Kim joined the organization in 2019 and has been the architect of C3’s work to identify and address health-related social needs through partnerships across the healthcare and social services sectors. She designed and implemented C3’s $25M Medicaid-funded Flexible Services program in Massachusetts which improves health outcomes and reduces health care costs. Kim is passionate about sharing these successful approaches to value-based care, partnerships and social health programs to expand primary care to advance health equity.

Prior to joining C3, Kim spent two decades addressing food insecurity and health with Feeding America through roles in public policy, nutrition programs, and healthcare partnership consulting. Kim is a Registered Dietitian Nutritionist with a BS in Dietetics from Purdue University and has a Master of Public Policy from the University of Chicago.  She lives in the Boston area.

Now as the Executive Director of Quality Performance and Value in the newly formed Primary Care Network, Athena is leading a team focused on capturing all aspects of quality and value to improve population health and patient experience, while reducing the cost of care through efficiencies in workflows. As an educator and public speaker, Athena consults with a variety of customers on optimizing workflows to maximize revenue while improving consumer and provider experience.

Athena has earned a Master of Public Health, along with a Master of Psychology and a Master of Management and Organizational Leadership. She recently concluded a Harvard course in Global Health Delivery, is an MBTI and PPS practitioner, AAPC certified in CPC, CRC and CDEO, and is an ordained minister. Dr. Silverstein is a national healthcare strategic thought leader with expertise in operations and governance. Dr. Silverstein was named one of the 10 people to know in the World of ACOs by Becker’s Healthcare. He received his M.D. from the University of Chicago Pritzker School of Medicine and completed his internship in internal medicine with McGaw Medical Center of Northwestern University.

As Director of Risk Adjustment, Susan Waterman has been empowered to plan, design and oversee business and strategic objectives in creating and optimizing a Risk Adjustment Department responsible for ensuring the accuracy of risk adjustment payments while successfully managing all activities related to Medicare Advantage, ACA and Exchange Risk Adjusted lines of business. In that capacity Susan directed department changes that resulted in multi-million dollar gains in Risk Adjustment, brought all chart review activity in-house, and partnered with the hospital CDI/Quality Physicians to create an Outpatient CDI Department focused on documentation quality, Risk Adjustment activities and clinic training.

A proven leader in her field, Susan’s professional experience includes coding and compliance management, auditing and provider training, system management, and consulting services.
